Astarion admits, when prompted, that he doesn't remember a great deal about his life before he was turned into a vampire spawn. He had been very young in elven years, and it's possible that the trauma of his near death and later transformation was enough to destroy any previous memories he had. What Astarion does remember is limited - he was born and raised in Baldur's Gate and became a magistrate judge. This resulted in a poor choice, leading to the Gur hunting him down and leaving him for dead, where he was later "saved" by Cazador Szarr, a vampire lord who enslaved Astarion for the next two centuries.

Being Cazador's slave was essentially two hundred years of torture for Astarion, completely subservient to his Master's control due to their vampiric bond. Forced to follow strict rules, Cazador sent his spawn out at night to collect 'hunts' for him - seducing people or kidnapping them in order to bring them back for Cazador to supposedly feast upon. In return, Astarion was only permitted to eat rats, could only obey his Master and was not allowed to leave the house without his permission.

For Astarion, the capturing of 'prey' was, more often than not, completed through seduction - visiting taverns, brothels and other places to seduce whomever he could find and take them back for Cazador to use as he saw fit - and it was assumed he was consuming them for food or sustenance. If Astarion failed to follow instructions, he was punished - such as being locked in a tomb for an entire year with no food, breaking his will and leaving him entirely broken at the hands of his Master. Throughout this time, Cazador also tortured his spawn, including carving infernal language on Astarion's back.

Astarion remained unaware of what the carvings in his back meant until he and his companions met with the devil Raphael, who informed them it was part of a ritual with the devil Mephistopheles, to grant Cazador the power of a vampire ascendant at the cost of all the spawn that Cazador had made in his time - at the cost of Astarion's life.

At the beginning of the game, Astarion has been kidnapped by Mindflayers, where he was implanted with a tadpole that should transform him into a Mindflayer himself, but this did not happen. Instead, it broke Cazador's control over Astarion and allowed him to communicate telepathically with other infected, enter homes, walk in sunlight, etc - essentially, Astarion could act almost like a regular human.

SPOILERS FOR ASTARION'S PERSONAL QUEST After speaking to Raphael, the group was tasked with finding the orthon lurking under the Thorm Mausoleum, with the promise that if he was killed then Raphael would explain the scarring to Astarion. Once the task was completed, he was finally told the nature of the scarring and the ritual that Cazador intended to carry out back in Baldur's Gate. Astarion was determined to take the fight directly to Cazador and enact his vengeance for two hundred years of torture and slavery.

On the way to Cazador's mansion, two of Astarion's fellow spawn found him - Dalyria and Petras, who confirmed the ritual was to take place in a chapel under Cazador's palace. Ulma, the leader of a tribe of monster hunters - and the parents of children Astarion had stolen for Cazador - told him that he would be given redemption if he was able to stop Cazador and take his life. The group then began their march to Cazador's palace.

The ritual was to take place under the mansion; the group went and Astarion was almost pulled into the ritual to be used to fuel Cazador's power. After Cazador was defeated in combat, his real body was found inside a coffin - and Astarion faced a choice: steal the ritual and become an ascended vampire himself, or kill Cazador and leave without using the ritual. After being reminded of his own strength by his companions, Astarion chose to kill Cazador and free the thousands of spawn he was intending to kill, determined to seek his own future without the blood on his hands.

vampire spawn
As a vampire spawn, Astarion suffers numerous limitations (without the tadpole in his mind): he cannot walk in sunlight, he cannot enter buildings without permission, cross running water, he canot survive without consuming blood (animal blood can sustain him, but the blood of humanoids is better) or disobey his master. These limitations are voided with the tadpole in his mind.
